On or after from and on or before to; either bound may be omitted.
EQ
Within the window from from to to; both are required.
NOT_EQ
Not within the window from from to to; both are required. For appointment dates, requests without an appointment in the window.
GTE
On or after from, which is required.
LTE
On or before to, which is required.
